In 1990 Brazil began to focus its mental health system away from a psychiatric hospital system to psychosocial community health centers. Between 2002-2014, Brazil reduce the number of psychiatric hospital beds by nearly 60% while increasing the number of psychosocial centers from approximately 1 per 500,000 to 1 per 100,000. At the same time, Brazil expanded its primary care system. Medical teams (comprised of a physician, nurse, medical technician and 6 community workers) are placed in clinics across the country. Each team is assigned to approximately 5,000 people. Nearly half of the centers in Rio have been assigned a mental health professional (often a psychologist) to support local treatment of mental health and substance use disorders. Dr. Fortes will describe the successes and challenges of ongoing efforts in Rio to expand local mental health services through training, quality improvement efforts, and educational campaigns.
